Unlike celiac disease NCGS, also known as gluten intolerance, is not a well-defined entity and surrounded with controversy in medical community.
There is no test or biomarkers to help with the diagnosis. No structural damage or decrease in absorption. No long-term complications or risk of association with other autoimmune disease.
Patient presents with GI Symptoms like celiac disease and respond well to a gluten free diet. It’s unclear if it’s due to the gluten removal or the diet that’s usually lower in processed foods and carbohydrates (fructans).

Wheat allergy, one the 10 most common allergies in adults in the United States, is an immune reaction to wheat and not gluten, which is quick and involve a range of symptoms from watery eyes, runny nose, nausea, abdominal pain, itching, hives to trouble breathing, racing heart and possible anaphylaxis. Patient need to avoid wheat but can tolerate gluten from other sources.
